- Letter of Instruction or Bank Information add form and pre-printed voided check or deposit slip. Letter and form require MSG to be processed. If voided check or deposit slip is unavailable, letter of instruction on Bank letter head with bank details can also be sent. This document will require an MSG and signed by bank manager or above
